Five ways in. One standard of care.
Rehab is one-on-one with a Doctor of Physical Therapy, on a plan with an endpoint. Performance training runs with a certified coach on the same floor, which is the whole reason the handoff between them works.
Physical Therapy
A full hour, one patient, one doctor. The cause-first evaluation everything else here is built on.
02Sports Rehab
Return-to-sport testing you have to pass, not a date circled on a calendar. Rehab that ends on the field, not on the table.
03Dry Needling
Precise work for the trigger point that has outlasted stretching and rolling. A tool inside the plan, never the whole plan.
04Massage Therapy
Recovery-grade soft tissue work that supports what your doctor is already treating. Not a spa hour.
05Performance Training
Speed, strength and testing with a certified coach, aimed at the injury that hasn’t happened yet.
You leave visit one holding the plan.
Most pain doesn’t need more mystery. It needs a cause-first evaluation: finding the reason, not renting relief from the symptom.
From there, care moves through four named phases, Pain, Prime, Perform, Prevent, each with its own job and its own criteria for moving on. You always know which phase you’re in and what earns you the next one.
And every plan has an endpoint. We’re here to finish the work, not to see you indefinitely.
Get my plan started ⟶Calm it down.
Hands-on treatment begins at the first visit. You leave understanding what’s actually going on, and you feel a difference early.
Restore what’s stuck.
Targeted mobility work for the joints and tissue actually holding you back, dosed to your body, not a printed sheet.
Build strength you trust.
Progressive load against standards you have to clear before moving on. Confidence, here, is a training effect.
Make it stay fixed.
Testing before you’re cleared, then a keep-it-gone program sized to your real week. Then we graduate you.
Corrections to the record.
Physical therapy has a reputation problem. It was earned, just not here. The record, corrected:
-
“PT is a photocopied handout and a heat pack.”
CorrectedEvery visit is hands-on and one-on-one with a Doctor of Physical Therapy. If a sheet of paper could fix it, we’d mail you the paper and wish you well.
-
“You’re one of three patients they’re juggling that hour.”
CorrectedOne patient. One doctor. One hour, on the clock for you and nobody else. Your appointment is yours; nobody shares it.
-
“Rehab is over when the pain stops.”
CorrectedPain leaving is the middle of the plan, not the end of it. Perform and Prevent are the phases where the thing that hurt you loses the ability to do it again, which is why we test before we clear anyone.
-
“Once you start, you’re in for as long as they can bill it.”
CorrectedYour plan has four named phases and an endpoint we name out loud at the evaluation, before you agree to anything. Graduation is the goal, not retention.
-
“You need a referral before anyone will even look at it.”
CorrectedYou don’t. Florida allows direct access: book the evaluation and we handle the paperwork behind you. No gatekeeper, no note from your doctor.
Your first visit, annotated.
One hour, one room, one doctor. Here’s the anatomy of it.
You talk. We actually listen.
The whole story, not the three lines that fit on an intake form: when it started, what it’s costing you, what you’ve already tried.
We test, not guess.
Movement, strength, and the joints above and below the one that hurts. Where it hurts isn’t always why it hurts.
Treatment starts today.
Hands-on work in the first hour, not the third week. You should leave feeling that something changed.
You leave with answers.
What it is, how long it should take, what phase one looks like, and what visit two will do. In plain language.
Wear something you can move in. Everything else is our job.
The people holding the plan.
Five Doctors of Physical Therapy, a physical therapy assistant, and a separate bench of certified performance coaches. Twelve people, one plan per patient.
Dr. Sharif Tabbah
Founder · DPT, PT, CSCS
Treats the evaluation like the first real day of care, not the paperwork standing in front of it.
Dr. Chris Thurston
Manual Therapy · PT, DPT, OCS, FAAOMPT
Clears athletes with test results, not optimism. If the number says not yet, it’s not yet.
Dr. Ryan Johnson

Hands-on by training and by preference. A joint that won’t move gets found out before lunch.
Dr. Sami Jamal

Runs the Prevent phase like a coaching staff runs a preseason: screens first, standards second, graduation earned.
“Shout out to our dude Dr. Reef. I’m telling you man, Sharif down here in Miami. We down here in Miami getting it in, every offseason, he never fails.”Alvin Kamara · New Orleans Saints
After tearing my hamstring during my conference meet, I was devastated and in desperate need of a physical therapist who could help me get back on the track. The staff made me feel welcome and confident that I would recover. Every day, they motivated me, encouraging me to trust the process.

The straight answer on cost.
Nobody should need a decoder ring to find out what care costs. Pick your situation; we’ll say it plainly.
We accept out-of-network benefits from every insurer, and we are in-network with Tricare at the Davie clinic. Either way, we verify your exact benefits before your first visit: copay, deductible status, visit limits, all of it.
You’ll get the numbers in plain English before you owe anyone anything. No surprise statements six weeks later.
Verification is free and takes about a business day. Use the form to start it now.
Haven’t met your deductible yet? We’ll tell you the exact per-visit number before you book, not after the first statement arrives.
Because your plan has a named endpoint, you’ll also know roughly how many visits we’re talking about, so you can do real math instead of hopeful math.
Every dollar still counts toward the deductible you’d be paying down anyway.
No insurance doesn’t mean no care. We have a self-pay rate, stated before you book, due at the time of service. Cash, debit, credit and HSA all work.
One-on-one care also tends to mean fewer total visits than a volume clinic, so most self-pay patients spend less than they feared.
Ask for the self-pay rate when we call to schedule. It’s a number, not a negotiation.
We’re on it.
Expect a call within one business day with your copay, deductible status, and what your plan covers, in plain English.
Questions, answered plainly.
The things people ask on the phone, written down.
Most PT that fails, fails the same way: a rushed evaluation, a generic exercise sheet, and a therapist splitting the hour three ways. Here you get a full hour, one-on-one with a Doctor of Physical Therapy, and a cause-first evaluation that treats the reason for your pain instead of chasing the symptom. Then the plan keeps going past the point the pain stops, which is the part most people never got the first time.
No. Florida allows direct access, which means you can book an evaluation without seeing anyone else first, and state law allows 30 days of treatment before a referral is required. If your plan needs one sooner for reimbursement, we’ll flag that during your free benefits check and help you get it.
It depends on your plan, so we check it for you before your first visit, free. We accept out-of-network benefits from every insurer and we’re in-network with Tricare at the Davie clinic. You’ll know your copay, where your deductible stands, and any visit limits before you owe anyone anything. If you’re self-pay, we quote the flat rate up front, due at the time of service.
Most patients notice a meaningful difference within the first two to three visits, because hands-on treatment starts at the evaluation itself. The full plan, through the Pain, Prime, Perform, and Prevent phases, typically runs several weeks to a few months depending on the problem. You’ll get a specific estimate at your evaluation, and we track progress against it out loud.
Sometimes the pain fades. The cause usually stays. That’s why it comes back the week you add mileage, play a tournament, or finally lift heavy again, and why each round tends to last a little longer than the last. An evaluation tells you what you’re actually dealing with, so waiting becomes a decision you’re making on purpose instead of a gamble.
Clothes you can move in, your insurance card if you have one, and any imaging or reports you already have, though you don’t need imaging to start. Arrive five minutes early and we’ll handle the rest. The evaluation, including hands-on treatment, takes about an hour.
Put it in writing.
Five short questions. A real person calls you back, usually within two business hours, with times that fit your week.
The people who end up filling this out are rarely in the worst pain of their life. They’re just done working around it, done picking the errand that doesn’t require the stairs. If that’s the stage you’re at, this is the part where you stop researching and start.
Consider it filed.
A real person will call you back, usually within two business hours, with appointment times that fit your week. Keep your phone nearby.
